Anganwadi — I want to submit a complaint against anganwadi worker

I have submitted my daughters birth in pothavaram, nallajerla mandalam, west godavari district. When i asked for my daughters registration book they replied that they have lost the book and said that what ever you want to do you can do. And they are not even informing about the regular medicine for my daughter. Please take the necessary action against them. The government has given 6000/- for my daughter to the anganwadi but still they have not reached us.
