Avadi Municipality — Death certificate

My father (r umapathi s/o s raju) was a stage 4 cancer patient and passed away due to caridac arrest on 6 oct 2016 at sri sudharshan hospital, avadi. the hospital applied for the death certificate on the same day. but till date, we have note received the certificate. every time we visit the avadi municipality, one or the other reason is been told for the their delay. we are already emotional down, above this we are being shuttled. if we don't receive the certificate in next few days, i may post on twitter to pm modi.
