Bajaj Finserv — To cancel cpp liv care service/Membership

I was recently taken "PLCS PORTFOLIO MANAGEMENT GROUP" Personal loan (with Loan Account No. P404PPM4910800) from bajaj finaserv ltd.,
And cpp liv care service worth 6999/-rupees membership was activated for my account without my authorization.

I want to cancel that insurance or the service which is activated.

My customer id: 12951565.
Cpp liv care policy Membership Number :-IL5568597

Please resolve the issue as soon as possible...
