Bank Of India — atm withdrawal failed twice

Dear sir,

I kanchan chaudhari tried to withdrawal cash of rs. 10, 000/- from icici atm id: sacwm706 on 09.12.2018 - twice, and didn't get any amount from atm.
My bank is bank of india, ssi branch, vapi, gujrat, my account no. Is [protected]. I had submitted charge - back claim form in my branch. But till date, i had not received or credit any amount in my account.

I request you to please do the needful as already 50 days had completed and no positive reply i had received from the bank.

Sir I'm Tajel khan my account no [protected] Sir, 26 January morning I tried to withdraw 300 from kvb atm and I had swipe ATM machine inside I have press atm pin and amount then mony deducted from my account but I didn't get the cash
