Bank Of India — money reversal

Sir, i need your support to recredit 5000rs in my account i. E. ([protected]) of gorakhnath gorakhpur branch. As i withdraw mentioned money but i didn't get money in my hand from "union bank of india premnagar" branch at date of 4-may-2018 and moreover mentioned money had debited from my account. I have already submitted money reversal form from boi ballupur branch.
Kindly requesting to you to recredit mentioned money in mentioned account.
