With reference to the above subject, I am policy holder of BHARTI AXA GENERAL INSURANCE COMPANY .My policy number is HPD/Q0077322/71/04

We had put medi-claim file for cash reimbursement on dt.[protected] at Bharti Axa TPA – Paramount Ahmedabad Office All the queries had been replied with supporting.

With reference to claim repudiation lette of Bharti Axa DM and HTN since last 5 years as per doctor shown in report so this is pre-existing disease At the time of taking policy not mention by policy holder as per letter.

But due to doctor misunderstanding wrongly mention in First Consultation report so all have mention in Form, Discharge summary so claim is rejected.

But I have already submit doctor confirmation certificate dt 25.02.2016 due to mistake wrongly mention in report but till insurance company not agree for same and DM and HTM is not corelation in this joint replacement operation also mention in doctor certificate .

You are kindly requested to look into this and do the needful at the earliest.
