I am writing to urgently request your intervention regarding my Provident Fund (PF) withdrawal claim, which has been rejected due to a discrepancy in my employment records from companies end.
To resolve the rejection, I raised an official grievance with the EPFO. However, the EPFO Regional Office (SRO Whitefield) has put a "Closure Proposed" status on it because they require documentation directly from the company.
As shown in the attached screenshot, the EPFO has stated that while my wages were under ₹15, 000, EPS (Pension) contributions were not submitted by the employer from my date of joining up to 01/2021.
The official EPFO decision states:
"Hence member is requested to contact employer and submit revised 3A for further necessary action from our end sir"

Required Action: Submission of a Revised Form 3A for the period from my joining date to January 2021.
Because my final PF withdrawal claim cannot be processed until my pension history is updated, this matter is extremely critical and time-sensitive. If the revised Form 3A is not submitted promptly, the EPFO will permanently close this grievance, further delaying my withdrawal.
Could you please prepare the Revised Form 3A reflecting the correct EPS contributions for that period and upload it to the employer portal as a matter of top priority?
