Charnock Hospital, Kolkata — Form-B not received with filled and signed stamp of Charnock Hospital

I m Vikash Kumar Jaiswal, patient id- CHPL23015991 discharged from hospital on 29.06.23 at 15:15:34.
I have submitted a Form-B along with discharge document via mail of TPA desk, [protected]@charnockhospital.com.
But still as of now I have not received the signed and stamp copy of Form-B.
It’s very urgent for me as I have to claim the expenses.
