CMJ University — regarding issuance of original degree certificate

My name is devendra rathaur i have completed my bsc final degree course in the year 2012 from cmj university but not yet received original degree when i approached to my centre they don't have forms to apply for convocation certificate and been told to try on my own there is no clarity in university website to obtain convocation certificate please help my registration no is ic[protected] my mobile no [protected] my email id [protected]@gmail.com
