Cognizant — not able to modify the pan self declaration in pre join formalities

I received my offer letter recently and i was asked to fill the pre joining documents. I filled all documents. After submitting the pan declaration form, i found a mistake on my name. So i clicked the modify icon, but after modifying the name as per the pan card i could not able to submit because the submit icon was not available. Please give me a solution.
