[Resolved]  CBSE — Lost 12th certificate and marksheet

sir,
i have lost my 12 th and 10th cerificates respectively, can you help me to obtain a duplicate certificate as i require very urgently.
i studied in B S MOOTHA SENIOR SECONDARY SCHOOL, CHENNAI and i have passed 10th and 12th in my first attempt,

I hav Passed CBSE-12th class examination in July-2009, roll-no 2687765 from Jawahar Nova vidyalyah, J&K -Baramullah. Nw i want my Marks certificate of 12th class. I consulted the concerned authorities but thay told me to contact the regional office CBSE, I did the same but of no avail. Now i am requesting u through this complaint box to please tell me the procedure how to get the marks certificate. Its very urgent otherwise no any university will allow me to appear for Ist yr examination over here.

Please note that, due to typical error by C.B.S.E Board, My surname wrote in the high school and intermediate certificates wrong.
The details of the same is following:

X th -C.B.S.E Board -Parmeshveri devi dhanuka, vrindavan -1996
XII th -C.B.S.E Board -Parmeshveri devi dhanuka, vrindavan -1998

Please tell me the procedure for applying to get the certificate and marksheet after correction . Also, please tell what all documents do I need to submit. I will be highly obliged.
