[Resolved]  Garima Jain — Not issuing passport

I would like to inform you that i have applied for my Passport seven month ago police enquiry has been cleared my file No.BPL-A026941 My Name is Garima Jain .Earlier the status was "paasport will despatch on 11th march 2010",but now status is showing "Please furnish 3 Personal particulars forms,duly completed for verification",please tell me what should i do?
Therefore i request to you kindly issue passport as your earliest posible for this i shall higly oblidge.
