BIRTH DATE : 24/08/1978
My name : JAVEDMIYA NURMIYA

It is right on Pan Card.

My Father name is NURMIYA
My Grandfather name is RAJUMIYA.

In Printed pan card My father name was RAJUMIYA and Grandfather name is NURMIYA

Dear Sir/Madam

We have a pan card having no. AYBPS5109F in the name of CHIRANJIV KUMAR but as in PAN card office records my name is registered as CHIRANJIV KUMAR SINGH instead of CHIRANJIV KUMAR
We are facing a lot of problems during certain verifications like demet accounts etc.

There is no any other changes is required in the PAN card.
