I HAVE LOST MY PAN CARD NO.AKPPK0747D.I HAVE APPLIED FOR DUPLICATE CARD AND YOUR OFFICE TURNED DOWN MY APPLICATION ASKING THAT MY SALARY SLIP WHICH I HAVE PUT AS MY ADDRESS PROOF IS NOT MATCHING BECAUSE IN MY SALARY SLIP THEY HAVE WRITTEN MY NAME AS AR KUMAR FOR THEIR COMFORT.SO KINDLY ISSUE A DUPLICATE PAN CARD ON MY NAME MR.
