Jal Nigam Lucknow — passport not received

respected sir,
i m shahzad husain rizvi, i m a government servant n i have submitted my passport form with NOC on[protected].
file no.-LKOG00046008
key no-[protected]
n i have yet to receive my passport, n when ever i enquire about my passport on net, this msg is displayed "police enquiry has not been received"
i also enquire to the police station and LIU but they said they have not received any file regarding me .
pls kindly concern my application n issue my passport soon .
