October 13, 2008 To The Branch Manager, ICICI Bank Limited Sector 16, Fardiabad Sub : Conversion of Life Time Super Pension Policy to Single Premium Policy with One time premium amount of Rs.2 lacs (already paid at the time of taking policy) Dear Sir, I would like to bring to your kind notice that I am a retired person and have practically no Income source to pay my subsequent premiums under the below mentioned policy :- Policy : LIFE TIME SUPER PENSION Policy No. : 06758132 Policy Term : 10 years Proposal Form Nmo. : 0972746 LOB ZONE : 00151827 BANK B 204 OERV FBM/FSC CODE : 00284142 LIM/CSR CODE : 7170368 In fact, my total family income from all sources is less than rupees 5-6 thousand per month and under such circumstances, I am not even in a position to support my family to meet both ends meet. Moreover, this income comes from my fixed deposit amount besides a meager pension amount of Rs.952/- per month from my PF. I was under the impression that in the said policy, I will get back money every year and pay back premium amount to continue my policy for 3-years since the persons sitting in ICICI Bank did not give me the clear picture. The name of such persons from whom I took the policy are as under :- ICICI Bank Ltd, Sector 16 Faridabad team comprising following members took me into their chamber :- 1) Mr Ankur Garg 2) Mr Sahil Malhotra 3) Mr N Jain 4) Mr Yogesh Including their Top Executive name I don't remember. Had I known all the facts, I would not have opted to take the above policy. Moreover, I and my wife are not keeping good health in this old age and even not in a position to take care of our health under some good medical practitioners. In view of the facts as explained above, I would request you to kindly convert my captioned policy in a single premium amount of Rs.2 lacs (already paid initially) at the time of time of taking policy and no further premiums are charged to enable me to reduce my financial burden.
