My Car Swift dzire registration no-UP-78 BQ 5023 is repaired after accident by ONE UP motors,Mahanagar.Lucknow .Claim is not settled due to non creation of claim form 64 VB ,the total payment is done in cash against invoice no-BR12004016 dated[protected] at M/S one up motors at Mahanagar Lucknow,The survey has been done by Mr.Amod Kumar of NIC.It was said to me that I will receive my payment from M/S Sumitra motors,Shahjahanpur ,agency where insurance has been done.I am still waiting for the reimbursement of Claim against policy no-[protected] .
