[Resolved]  Pan Card — pan card lost applied with uti tsl

Application filed (grivance type) with UTI TSL for duplicate pan card on 28 March 2009 vide application No.[protected] Fee No.[protected] PAN NO.APPPS7431F - received a letter from UTI TSL stating father's name mis matched with original application.( Father"s Name:PUTHAN HOUSE VELAYUDHAN NAIR in original application - by mistake in grivence application mentioned as P.VELAYUDHAN NAIR) Based on the letter, the copy of 49 application(NINE YEARS OLD) sent to UTI TSL and asked them to send the PAN CARD based on the reference given but so far not response from them. Hence, you may verify and arrange to send the same at the earliest.

myself DATTATRAY KADAM from mumbai andheri east. my pan card no. is BIPPK1133G. i have lost my pan card
so i requst you to send me procedure to apply for same pan card. thanks.

sir,
with due respect i beg to state that I had applied for duplicate Pancard No. in favour of my Pancard no. ADNPN4873A ON 17.02.2011 which was wrongly mentioned as ADNPN48738 at my earlier application .I, therefore, requiest you to kindly issue the duplicate pancard at an early date.
