Pancard — surname wrong

Dear Sir,

I just received my pancard before few days. I come to found the serious mistake in my sir name .

It has been unfortunate to receive such serious errors despite of proving my documents upto date.
actually it is correct splled in pancard but in data base it shows wrongly.please go through and do corrections in your database.

the change you have to do is,
my name : venkata ravi shankar nemalikanti
but it shows rachapudi .
kindly do the corrections.

i have applied for a pan card which is delivered and reached to me.but unfortunattely the date of birth is written wrong.what should i do for that.my pan card no is ALEPK9527D.
