[Resolved]  Provident Fund — withdrawal of p.f fund

Dear Sir,
I was joined the icici pru. life insurance co.on 20th aug.-2008 & Resigned on 13th july2009. After resignation company did not send my form-16 or PF form. thats why i am unable to withdrawal my PF amount.
My employee ID was 562411 & designation was Asst. manager-agency development, Udaipur (rajasthan).
Please help me to get back my Provident fund amount.

Provident Fund — Withdrawl of PF

Ref : PF NO: DL/2293/03680

Reply still awaited and dues pending of last 22yrs. Earnest request if the dues can be given to me at your earliest as urgent need of daughter's marriage.

I shall be extremly grateful if this can be done on urgent basis.
