Provident Fund — PF Claim not settled

Hi,

I have submitted my Provident Fund form duly filled to Regional PF Commissioner Office, Mumbai on 30.03.2009 for final settlement of PF Claim. Three months already passed but the same is not yet settled (since the last employer is not signing the PF Claim form). My PF account no. is MH/40341/04.

I don't know how to get my PF amount settled or to whom should I contact so that the same will get settled at the earlies.

Please help me in this regard,

I Mr Arjun Singh Rawat had applied the matter about my provident fund back in november 2010 in PHL Pharma P ltd(now Abbot True Care). But still i haven't recieved any of my P.F. amount. So I kindly like to request you to take concern of this matter for which act of your I will be humbled. my P.F. no is MH/210550/003009
