Regional Passport Office Chandigarh — not received the passport

Sir,

I have not received my passport yet, i request you to please provide me with the status of it. I will be highly thankful to you for this kindness.. Z 817504, , and key number is :- [protected]...

I have applied for passport through Sapark Centre Sector 23-C, Chandigarh vide file no. Z-805997 on dated 28.06.2008.but till date I have not recieved my passport. Please issue me the same immediately as the same is badly delayed.

I have given my passport for name change, spouse name add, n changesin home address, I have not received the passport.we have sumit the application 06, may, 2009 on tattkal, they said they will despatch it on 08 may, but till dae i didnot get it, please do the need full.

Is this necessory that who want to make passport than he submit file by himself direct in Chandigarh office ?
OR
someone else (relative) can submit file in chandigarh office ? ?
