[Resolved]  Voter Id Card — Surname inscribed wrong in the ID card

I'm a resident of Nischinda, Dewanchak, P.O.-Ghoshpara (Bally), P.S.-Bally, Dt.-Howrah, State-West Beangal. In the year of 2007 I became eligible for inclusion of my name in the Voter List (my date of birth is 15.08.1988) and accordingly applied for the same under Domjur Assembly Constituency.
At the time of collecting my Voter ID Card I found that my surname as well as my father's surname has been inscribed as Panja instead of Bhaumik in the ID Card whereas the Electoral List bears the correct surname of myself and my father.
As a result of which I did not collect my ID Card and submitted Form No. 8 duly filled in as advised by the then officials concerned entrusted with the job.
Subsequently I was called to attend a hearing on 08.12.2007 which I attended.
But till date I 've not received my corrected ID Card. In spite of my repeated visit to our Local Panchayat Office namely Nischinda Gram Panchayat and The Office of The BDO of Jagacha Block, Howrah I failed to get my corrected ID Card. On the contrary the said offices could not trace my earlier wrong ID Card.
I furnish below my Electoral Details as per Electoral List-

Please advise as to how can I get my ID Card before the ensuing Parlimentary Election.
