Dena Bank — wrong deduction and not credited

Respected sir/madam,

Name : pushpaben arjunbhai makwana

When i withdraw 10000 from atm machine of finecare mini financial bank rajya chokdi rajkot, first try amount was not come out from machine and money was debited, after that money credited in my account,
After that i was try second time, money debited from my account but no money came out from machine,
This incident is held on 5/5/2018, 1.00 pm to 1.55 pm
That's why take action on this incident, and credit my lost amount in my account

very poor service dena bank
Every month my account gets debited with Rs 11 at least 10 times a month. Please provide a specific reason as to why these deduction are done without my consent
