Dear Sir / Madam
I had got my wife Arya bala evaluation done at your Lab under Lab No[protected] on 15 Aug 23..and till date the evaluation is partially complete, and in previous incident the lab report of my Daughter's Shreya Panda evaluation done at your Lab under Lab No[protected] on 15 Aug 23 was incorrect, and no fresh sample collection as advised by Lab Coordinator ex HO has been coordinated till now, despite telling them the urgency.
