Employee's Providents Fund Organisation [EPFO] — Incorrect name and father name

Dear Sir/Madam,
I, Chingtham Sunilkumar Singh, an employee of kmbhss, Manipur. My name is incorrect in EPFO record when i check on epfo website. When I try to update KYC, Aadhaar link but not success due to not matching the name and my fathers name. So I post correction form to the regional head office (Guwahati) but not corrected yet since 2016 (again recently submitted on 27th July 2017). so please look into the matter.

Hi am with UAN no[protected] ..I ve submitted my withdrawal form on 2nd June 2017 I ve akmowledgement letter also but office person is saying I ve not submitted..pls suggest me what to do ...this is the 3rdte my application is getting rejected
