HCL Technologies — required annexure-k for pf transfer

Hi,
I am an ex-employee of hcl tech. And requested for a pf transfer and it is showing a settled in epfo website.

But in order to transfer the amount to my pf account, i need a annexure-k from hcl pf team.

I've been following up with pf team from a very long time but they are not responding to my mails.
