Icici Lombard — haressing for an extra document which does not mentioned in claim form

I have my corporate medical claim and against it i have taken treatment and claimed my all bill for medical claim.. O have attached all of mendetary. Document which is required and mentioned in medical from.. Then also after some days they call me and told me that there is no any discharge summary, but it was on main file then also i have requested doctor to make another one.. Then again after 15 days they say me to submit medicine detail like manufacturing date expiry date and batch number.. That was also given to them with file, then after communicating they told ok.. And again then they are asking me for physiological statement of physician which was not all mentioned on claim form to submit, and if it is required they must say me at first call in one time again and again they are delaying claim and it passed 2.5 months sir /madam my claim. No is[protected] and policy number is 4016/106/154381/01/000
