Dear sir/madam
This is my 3rd year of online insurance of my 2 wheeler regd no ka 03 eu 2557. Insurance no 3005/w[protected]/00/000 for the period of 24 aug 2017 to 23 aug 2017.
Every year i have to face problem for non receipt of hard copy of insurance. After so many correspondence i was able to receive the hard copy of insurance every year. Main problem i had found that you are sending courier to my very old address. You are issuing certificate mentioning in the new address but every year you are sending to old address as i track and found the address mention in the courier address.
You are requested to please check the proper address and send me accordingly at the earliest, my present address is as under:-
Verma s. P., #206, good wood apartment, 7th cross, vishwapriya nagar, vishnuvardhan statue road, begur main road, bangalore-560068

You are requested to send hard copy of my 2 wheeler insurance copy at the earliest under intimation to me.
