Hi Sir,
I had applied on UTIITSL website for correction in my name on PAN card, to change it from : SUSHANI SHIVAJI KADAM to SUSHANT SHIVAJI KADAM on 15 Nov 2019. My father's name is SHIVAJI RAMCHANDRA KADAM. But during the application process, on application form my father's name appears as 'SHIVAJI KADAM KADAM'.
My PAN CARD number is : AWQPK5172M
So I had sent email to utiitsl.gsd at utiitsl dot com containing an application for this correction on application form with my signature on 28 Nov 2019 ticket number of which is 741250. My email id is sushantkadam75 at gmail dot com.
Since I am denied 3 to 4 registrations just because of the wrong name on PAN card, it is seriously affecting my progress in some fields. When I had applied for frst time and this time also on application form the name was written rightly but it kept on changing on next screen. I do not want to pay for mistakes which are not done by me. I have contacted UTIITSL on their helpline number many times. But they say your mail has been forwarded and they advice to wait.
But after reading complaints here, I have very low expectations from anyone in this regard. But I need PAN card very urgently. I hope you understand its importance on time. Was this information helpful? | 
I had applied on UTIITSL website for correction in my name, my father`s name and date of birth on PAN card, to change it from to IMTIYAJ ALI ANSARI to IMTIYAJ AHMAD on 20 FEB 2025. My father's name is MUHAMMAD HAFIJ.But, IN PAN CARD HAFIJ ANSARI. It has to change MUHAMMAD HAFIJ.
My PAN CARD number is : BNQPA8582D
