I am Vaskar Piplai, residing at flat no.55/9, East Sinthee Road, Fakir Ghosh Lane, DumDum, Calcutta-700030, having consumer no.25025 against LPG ID [protected].I submitted form no.4(for non aadhar based LPG subsidy transfer) duly filled to my Indane Gas Distributor named M/s.Dum Dum Gas Supply Agency, Kolkata-700074 on 02.04.15 with due acknowledgement from them. The same has not been processed by the Distributor and I am yet to receive the subsidy amount in my Bank account.
Apart from that I received message in my mobile phone(registered with you) that on 13.03.15 at 10.08hrs.gas booking no.801102 has been generated, although it was not booked by me, on 18.03.15 at 17.27hrs. cash memo no. [protected] has been generated and on 20.03.15 at 18.28hrs, gas cylinder has been delivered.
It is apprehended that my gas account has been tampered by some un-authorised person for getting the benefit of subsidised rate of gas. It can reasonably be concluded that without having the connivance with the distributor such type of occurrence cannot take place. Kindly investigate the case critically and take necessary preventive measure after identifying the person involving such type of forgery under intimation to me and also intimate the detail of the cylinder booked/delivered to me from 01.01.2015 till date.
