[Resolved]  Indusind Bank — Delay in settling claim of Joint FD after death of 1st Holder.

SUBJECT: Withdrawal of Fixed Deposit on turning Major (18 years) & Death of 2nd Holder.

This is to state that my father invested in a fixed deposit at Indusind Bank in my name. The details of the FD are as follows:
Name: ALICIA.
Address: HOUSE NO 484, GUIRDOLIM, ANTHOI, CHANDOR SALCETE 403714.
Contact No: 2784231
Subsequently the Fixed Deposit had standing instructions of the Principal Amount + Interest Accrued to be reinvested and hence the Fixed Deposit was renewed with the following details:

Name: ALICIA.
Address: HOUSE NO 484, GUIRDOLIM, ANTHOI, CHANDOR SALCETE 403714.

Rs 19, 073.86.15 Maturity Date:[protected] Maturity amount: Rs 20, 831.64.

What was not conveyed to the bank at the time of renewal was the fact that my father passed away on:
04 August 2015 at Goa Medical College.

Also during this period, I the undersigned of this application turned 18 years of age on 15 April 2016 with my Date of Birth being 15 April 1988 and hence turned Major and I am able to operate this Fixed Deposit account by myself.

The Fixed Deposit Certificate has a few errors that need to be addressed with regards to my father’s name which needs to be rectified, since it appears the same to be a hindrance in encashment of the Fixed Deposit.

i. My father’s name is mentioned on the Fixed Deposit as Tony Fernandes while his name on my Birth records read as Antonio Joao Inacio Fernandes.
ii. My father’s name is Antonio Joao Inacio Fernandes and is alias as Tony Fernandes.

Attached to this application to authenticate my father’s, I have attached the following documents:

My parent’s marriage certificate.
Father’s PAN CARD with number AADPF5157A.
Father’s ECI identity card stating his name as Tony Fernandes.
Aadhar card with registration No[protected].
