Respected sir,

I sachin kumar singh from west bengal working as financial advisor in sbi life since 12 years. Please change the status of my pan card as it is showing blacklist in pan look up. As there was big problems that i was not working with one company manager where he taken steps to spoil my career.in my career i have done 6 times mdrt where i used to get commission. My all customers are from paramilitary officer....

This is not much of a complaint but more of a suggestion.

I am a health care provider for ophthalmic services. When we are sending the preauth. documents to the TPA, they are invariably asking for a A scan report.

A scan report is mainly a document that surgeon needs to decide the power of the IOL tot be implanted, which he is entitled to change even at the time of surgery. It has no relevance for insurance companies...

I have Resigned in Kotak Life and got NOC for the same. I get NOC from them. My Pan card is still showing on IRDA portal. please remove the same.

I am NEERAJ MISHRA i want to deactivate my agent code from irda portal. Agent code is ICI01424180 and my pan no is CNRPM9613F. Because i got another job. I already got terminated from the lici. I also enclosed my termination letter below.

My agency Code 7111378Q of Life insurance corporation of India is still showing in force on IRDA PAN LOOKUP PORTAL.
Application has mailed to @irdai.gov.in on the month of September, 2020, for removing my PAN but I didn't get any replies from them and the same is still showing till date.

It's my humble request to you please remove my agent Code from IRDA PORTAL.

My agency Code BA[protected]of Star health insurance company Ltd is still showing in force on IRDA PAN LOOKUP PORTAL.
After several times follow up to branch where the application has submitted on the month of October, 2020, and also mail to [protected]@irdai.gov.in for removing my PAN but I didn't get any replies from them and the same is still showing till date.

In my pan card no showing active LIC agency from 1st april 2015 on IRDA Portal. seens 2015 am not done business for LIC .
