Irda — Cancellation of my agency code

Dear sir/ma'am

With reference to the above I have to state that I have submitted for termination of my caption agency code to Religare Life office, Guwahati, and handed over my application to Mr.Pankaj gogoi(sales manager) and Pulok Debnath (Branch manager)together with ID card.But both is delaying the process at the pretence of one or other.Mr.Debnath is now asking to hand over welcome letter, which I could not find out despite of my best efforts.
Therefore, I request you to issue necessary order and instructions to to the Branch Manager Religare Life, Guwahati to terminate my agency code from IRDA portal with immediate effect.
