Irda — Please remove my pan card

My name is Tarun Bhagotra and my pan card no is -DAMPB5345A my agent code is BAL256799. My code is already terminated but showing in IRDA portal in bharti axa life insurance new company not allowed in any agent code in any insurance company.so please deactivate my agent code please the status of remove in irda portal.please revert my mail as soon as possible
