High networth insurance (Jio hni) mediclaim policy was issued in feb'16 in the name of kushagra parakh through oriental insurance co, bearing no. 530000/48/2016/507. After issuance of cashless facility was extended for first four months but since july'16 cashless facility was withdrawn and tpia (Health india) asked undersigned to take reimbursement by submitting claim forms for remaining treatment.. Duly completed claim forms (3 nos) along with all relevent documents were submitted to health india office at anand commercial compound, gandhi nagar, vikroli, mumbai on dated 23rd july 2016. Further 2 more claim forms duly completed along with all necessary documents were sent to health india vikroli office. During scrutiny of claim forms, deficiencies as intimated by health india thru email dated 04.09.16 were also taken care and all requisite documents were sent to tpia thru blue dart courier in the month of sept'16. Claims of about rs. 1.80 lac are lying pending even after elapse of more than 3 months.
