Jammu & Kashmir Bank / J&k Bank — jammu & kashmir bank / j&k bank amount has been debited from my account but declined from swiping machine/device transaction

Dear bank,

I am sharavana kanniyappan s/o r kanniyappan (Inr) -[protected] holding j & k bank global debit card and card no. [protected] mastercard and it is my salary account. I was swiped for money 10, 000 on 17/05/2019 at naryana narayana nethralaya eye hospital, near iskcon temple, 121/c, chord rd, 1st r block, rajajinagar, bengaluru, karnataka 560010. But that transition has been declined and given error, but i was checked that amount has been debited from my account. So kindly please let me know about how file complaints for refund and further processing.
