This is to inform you that i have 190 physical shares of reliance industries ltd in the name of my mother and myself the details of the shares are as follows

3) 95 shares

Certificate no 62617760
4) 25 shares

Certificate no 16660763

I had submitted the same on 8th october via my icici direct account (Acc no [protected]) for dematerilization of the same i had included the following proofs
Notarized death certificate of my mother
Proof for signature from my bank manager with details of my bank account
Notarized affidavit for my signature
Even then the shares have been rejected for mismatch of my signature and till date i have not yet received back my physical shares

I have sent multiple emails and have called Karvy but not getting the resolution on below problem. I have physical share certificates jointly held by mother and father. Father is no more and want to make my mother single holder of shares and dematerialise it. What is the process to be followed??
