Kotak Life Insurance — Premium receipts not received

Policy No.02733961 in the name. Of PKarthik premiums paid through ECS during the month of October 2015 and January 2016 were not received by me. I am in need of these receipts very urgently. Pl mail it to my mail ID [protected]@gmail.com.

My Policy no is 02355610 and Client ID is 54645017 in the name of Tushar Bhuruk premium paid through online payment amount of Rs. 20450/- where the receipt of this premium is not received by me. I am in need of these receipts very urgently. Please mail it to my mail ID [protected]@yahoo.co.in.

Policy no.01391326 and client no is 52249404 in the name of POOVAIAH.K.K paid premium on line for Rs25, 000 about 20 days back. I was informed by pop up message that I need to submit one formed filled and submit to the branch related to some USA tax protocol.Though I am a resident Indian I was told to submit it as it is compulsory. I have filled and submitted the same at HSR lay out branch. I have asked for paid receipt in the branch but one person introduced himself as Chief manager Raghunath Gundluru told this branch being new one and infrastructure was put in place to generate receipt but promised same would be sent to me via mail /post immediately.
But so for it has not come to me. Further I am so much disappointed the way the services provided by the Kodak insurance.
Pl.send the receipt to my registered E-mail ID.
