Lenovo — Received another phone and issue not resolved- need a replace or refund

I have given my phone for servicing on 15/06/2015 at "Lenovo Smart Technologies, Panampilly Nagar, Ernakulam, Kerala." Complaint was regarding the network issue. They asked to change the board and it was not yet resolved. I received the mobile back on 06/07/2015, but with a damaged panel, screen card was removed, screen was dirty, and the issue is still remaining. I am sure the panel or the mobile is not mine because there are scratches on the battery.
