Respected sir,

1. I wish to submit the following few lines for your kind consideration and favourable action please.

2. It is to inform you that my husband mr. Mahendra pratap singh had shot dead by criminals on 13 jun 2015. My husband had taken a lic policy no[protected]Marriage endwt/educn annuity with profit with dab) on 25 aug 2005 which was matured on 25 aug 2015.

3. After death of my husband i was liaised with the staff of the office of lic of india, district – sultanpur alongwith ibid lic policy bond on 01 jul 15. Staff of the ibid lic office handed over some claim forms to me and advised that to submit the ibid claim forms to this office duly completed for payment of dab.

4. On 12 jul 2015 i was again liaised with the staff of the office of lic of india, sultanpur and submitted the claim forms duly completed along with the other related documents in respect of my deceased husband which were as under:-

(A) self application.
(B) death certificate of my late husband in original.
(C) self attested copy of postmortem report of my husband.
(D) self attested copy of first information report at police station gosaiganj, distt - sultanpur.
(E) original lic policy bond.
(F) lic of india form no 3783 (A) revised – 2008.
(G) lic of india form no 3785 (Revised)
(H) lic of india neft - mandate form.
(J) self attested copy of my bank passbook.
(K) lic of india form no 25/1.
(L) copy of report by police station gosaiganj, distt - sultanpur.

5. On 24 sep 2015 an amount for a sum of rs 64, 063/- (Rupees sixty four thousand sixty three only) credited into my bank account from lic of india on account of maturity claim of lic policy no[protected].

6. On 10 oct 2015 i again met the staff of the office of lic of india, sultanpur and informed that an amount for rs 64, 063/- (Rupees sixty four thousand sixty three only) has been credited into my bank account which is the maturity value of the ibid lic policy. What about the death insurance amount of rs 1, 00, 000/- which should be credited into my bank account. On scrutiny of the case file in the office computer staff of the office of lic of india, sultanpur told me that maturity value of the ibid lic policy has been paid to the nominee of deceased vide this office payment voucher dated 24 sep 2015. You are not entitled for death insurance claim..

7. I told the dealing staff of the office of lic of india that the lic policy no[protected] in respect of my late husband which was matured on 25 aug 2015. My husband had shot dead by criminals on 13 jun 2015 and document for dab claim had submitted to your office on 12 jul 2015 before maturity date of the ibid lic policy, which is not understood, and why you are not paying dab claim to me.

8. Dealing staff of the office of lic of india, sultanpur again told me that you are not entitled for dab claim of the ibid lic policy. I met the branch manager of the office of lic of india, sultanpur and briefed the complete case. Branch manager immediately called the dealing staff, asked to brief description of the case, and requested me to give them 10 minutes time so he can talk about the matter with his dealing staff. After getting the brief description of the case by the dealing staff, branch manager told me that the statement given by the dealing staff to you is correct (I. E. I am not entitled for dab claim of the ibid lic policy).

9. In view of the above, you are requested to examine case of ibid lic policy and pay death insurance claim of the ibid lic policy which is entitled to me.

10. I shall be highly grateful to you for this act of kindness.
