Max Healthcare — doctor refusing to share registration number and forcing for a new medical checkup

I have already had a checkup with a lady doctor at Max Health Super Speciality center at Saket, Delhi. No where in the OPD card of prescription has she mentioned her registration number which I need to sumbit to my office for medical claim. Her PA is completely useless. The doctor is forcing me to either do a new medical checkup then only she will provide the registration number or else she wont provide the number. She is intentionally trying to loot the .money just for a registraion number. I have already had a checkup thie May 2019 but she is forcing for a new checkup.
