Dear sir,
I'm jayanthi phani kishore, d. No.2-99/7a, madhuranagar, kakinada. I have approached your municipal corporation death & birth register regarding my son death certificate which is happened in 03.05.2011, we have received death certificate with of father and mother & other details wide death certificate register no.3094 & dt.09.05.2011, for correction of name & other details. If we asked aso, concerned person's finally they found that in municipal office records details not found, this is informed after 15 days from date of approach by concerned person and advised me to approach, ggh for those details confirmation. Ggh mrd section information those details with yellow copies already sent to kmc on dt.11.05.2011 wide their internal ys no.1420, op no.15715, those records misplaced in municipal office.
Finally lot of struggles and mis communication, irresponsible answers from concerned department at municipal office about 1 month, get the details from ggh, kakinada officially and provided those records xerox with certification provided my self to municipal office concerned persons to up date the system. My self also application forwarded through e-seva on dt.08.01.2018 wide udc[protected], provided relevant documents originals to municipal office on same date as per aso sir, instructions for further process.
Again your employees still behaving irresponsible manager and expecting some amount to fasted to process due to it will take time further.

The same certificate needed for my wife's maternity leave, which i pending in same municipal office with c4 section due to my wife working as sgt in dairy firm mcps, kakinada, & this is 3 rd pregnancy and at present we have 2 live childerns and applied for eligibility maternity leave as per maternity g. O.

We are very much struggling with your municipal office staff, i think they are intentionally delaying process, please look in to this and do the needful.

I have been complaining about street light since many months. Even though we called the electrician who is responsible for street light repair he said he would do that but since many months he did not finish the work and people have to walk in the darkness. Even though the road was not are cc road. It is situated near christian function hall. So i request the concern departmental authority to replace the light or repair it. As soon as possible. We are send the picture below.

We the people in meka narayanamma thota, jagannaickpur in a new layout 90 percent of cc roads were formed but just one road is not form since 6 years. Reason we don't know. We have sent letters to the concerning engineer several times and met him personally and he has visited personally but there was no out come of it. Please kindly take necessary action.

In my village so many dogs and pigs are disturbing people at night times and day times also.. Pepole are injured by them. So many accidents happened in rayudupalem due to pigs and dogs.. Each and every street having 7 to 10 dogs and pigs. So please kindly find us problems and rectify it as soon as earier

For the last 4 years we have been asking the assistant engineer mr. Bhavani sankar but each time he ask me to give the letter which i did for at least 3 times. But there was no use. Most of all the roads around this street were cement roads. But this is not fixed it in 4 years. So i request the engineering department to do the needful.

I, mr.g.visweswara rao resident of 68-9-33/1, ashok nagar colony kakinada, had applied for a new municipal water connection and avail the approval with order number:-[protected] up on this the respective department took the activity and made the provision on 8-8-2017.after the completion it was found that the provision made for the convience of end-user is inoperable.also we suspect the connection is made from the header having no flow of water.many efforts were made but in vain.the photos of the same are attached here with.

Hi,

I have seen a water pipe leaking on the jawahar street, kakinada opposite to ramalayam temple beside a juice center. The water pipe is leaking at the drainage line before an old building. Please fix the leakage and prevent water wastage.

Also, people informed about the water leakage few weeks ago to the municipal corporation employees but they have asked to register a complaint online to fix the leakage.

I am b v ramana rao, resident of triveni sangam apartments, venkatnagar. We are not unable to stay in home due to poor sanitary at cross road accumulated garbage. Last week some of my company staff (Japanees), they have commented about the kakinada smart city. This is very shame to our kakinada people. I am requesting the sanatoria inspector to visit the location and stay some time, how he can feel. Your necessary action to be take as early as possible.

Drainage works are troubling the citizens a lot, there is proper passage to come out from the home. Huge drain slush is stacked since 3 weeks, large number of misquotes are attacked in and around streets, there is no care about the treatment for misquotes, request to advise concerned to take care of citizens at rajendra nagar, vegullapapayya street, opp govt school

People are facing the problem of orphan dogs and their stray on the main road. They are in numbers. Their groups are dangerous as they started attacking the passerby. They run towards motor bike which lead to many accident in the area. People lost their sleep as dogs bark whole night. This problem is headache as person who has night shifts facing worst situation.

I am requesting you to look on that matter as soon as possible....

Sir, here the municipality persons are not cleaning drainage mud properly and they spread the mud all over the surrounding houses for that the mosquitos and other insects are spread the diseases and even we are unable to breath air because that smell. We are the Residents of SUBHAM VENTURES, Rameswaram Panchayat, Pedapadu Mandal of Kakinada. It is a Approved Lay out and more over we have already paid the amounts for the fixation of Street Lights also. Till today no one can come and fix the street lights. It is a rural area and more over robberies are continuously going on if, any one lock in the house for 1/2 an hour time not only that some snakes are also moving at the time of night. So we request the Kakinada Municipal Authority please kindly depute concerned staff for fixation of Street Lights. it is our humble request.

I am resident of kakinada. I have seen one accident opposite to the apsp(kakinada-pithapuram road) . I have observed that the main cause of the accident was lack of proper illumination on road. There are so many pits and manholes on that way. I found that without proper lighting system it is very dangerous to drive or travel in night time on that road. There are many such accidents from last few years. And i have learned that good amount of money was sanctioned for the kakinada as a part of smart city initiative. If we would have spent part of that money on street light system sure we would saved so many life's. I am sending the same copy to PMO, Mo[censored]rban development. I[censored] ignore this message i will keep this in public domain. It is very serious issue pls take proper action immediately.

It has been noticed that a wide incomplete drainage (Not even sure whether it has been completed of not) has been dug without slightest bit of information or notice to the residents staying here. Due to this the water is stagnating, causing bad smell and breeding a lot of mosquitos. What's the point of a drainage if the water does not flow? Also they have just put all the rubble aside without of cleaning it up. They also spoiled our water and gas pipes. Is this how you all handle contracts? Is this what you all call a smart city and swatch bharat. How are the residents supposed to live peacefully with all this. Now we have to build our own ramps again. Can you all please come and look at the situation and do something about it immediately.

Sir the municipal workers have not been cleaning the drain since it was constructed almost 4 month's which leaded to accumulation of water due to obstruction to drain. Water has been stagnated and which leaded to growth of FROGS AND SNAKES IN THE STAGNATED WATER AND ALSO LEADING TO GROWTH OF MOSQUITO S WHICH MAY LEAD TO HEALTH HAZARDS SO I REQUEST YOU TO LOOK AFTER THIS ISSUE

It is very often seen the street lights being allowed to keep on illumining even in day time by simply wasting power particularly in RamMohanRajaNagar, Kakinada and neighbouring places. Responsibility is not seen fixed on the concerned supervisory staff of Kakinada Municipal Corporation. It shows the callousness of the Supervisory staff and the Higher ups in the Corporation. In times where the street lights do not function, the concerned are not bothered to look into. The attitude of getting complaints from somebody and then attending on those complaints alone should be avoided. When the whole staff of the Corporation is moving out either for execution of any works or for routine rounds of supervision, excepting those ministerial staff, are all needed to pay attention on civic facilities as their duties and functions, to have good relations with the general public, as the staff are afterall working on payment of salaries collected from out of the taxes being paid by the citizens. All the staff are needed to function particularly in Urban Local Bodies by understanding the very purpose for which the Local Bodies are intended to.

Re:1) street dog disturbances in our street
There are 7 black dogs creating disturbance during nights and biting the people. More than 3 people suffered. Please take suitable action immediately, as we the residencies are facing terror to reach to our home especially during the night times.

Despite of application for water connection and having the line availability they are not proving the water connection.

We are struggling due to water problm and all these days (10 days) they escaped saying pipes are not available in the vicinity. But now the pipe availability is there so we applied for connection. Still they are delaying

Hi,

I have two kids born in private hospital of kakinada Municipal Corporation, Andhrapradesh.

Complaint Details: My first Kid was born on 16/11/2012 at "Ramya hospital, Gandhi Nagar, kakinada" and I applied for birth certificate in kakinada municipal office after 3 weeks birth of my kid and it was not done and they said they haven't received any record from hospital then again I went to hospital and got latter from hospital and submitted same to municipal corporation office and again applied for certificate but no use. then again Applied with lawyer affidavit through me-seva center and they are saying that "still it is not done".
I been trying for 3 year for my first kid and I am struggling get birth certificate from municipal office.

Recently my second kid was born on 8/10/2015 at "Trust Hospital, Sarpavaram junction.., kakinada" and hospital guys said that you can apply for birth certificate after 2 weeks but again same problem repeated which was happened to my first kid ..i.e. me-seva center guys are saying that we haven't received any records from hospital but hospital people are saying that "they already sent record and keep the record with us no use"

I observed and checked with my friends they are saying that "Whoever born in General hospital they are getting birth certificate and whoever born in private hospital they are not getting"
