Dated 21.8.2019 Sub: denial of claim no nicdr2/218069. Sir/madam, I daleep kumar dhar holder of national insurance company medical policy no 360203/50/18/0006130. I was diagnosed a chronic medical renal disease patient last year in the month of august and i was admitted to yashoda hoospital ghazibabd under medical claim which you had paid to hospital. For the dialysis i was advised by the doctor to have av fistula surgery in my left arm. I approached aura clinic janakpuri and got the surgery done and i submitted the claim for rs 16288.00 vide claim no nicdr2/200208 dated 12.9.2018 and i received the payment of rs 16096.00 by neft Now after some time my vain was not developed and again doctor advised me to get the transposition of vain surgery and i approached the aura clinic janakpuri and got the vain transposition surgery done and i submitted the claim to park land med claim vide claim no [protected]@/218069 dated 15.5.2019. After so many reminders my case was dismissed and denied. I got information from the reliable sources that my case was denied because the hospital from where i got the surgery done was not registered as per park medical rules. But i would like to say the when i got the av fistula surgery done from the same hospital vide claim no nicdr2/200208 dated 12.9.2018 was it registered at that time and now when i submitted another claim of transposition of vain and now they say it is not registered.
