National Insurance Company — non receipt of insurance policy & tp card

I have taken medi claim policy from your company since 1996 & till date without any claim. This year i have renewed my policy (Policy no 422100/48/16/[protected]) via cheque number 778321 dated 28/8/2017 pnb yamunanagar, deposited at you office of national insurance company ltd located at fountain chowk yamunanagar haryana. Pan number-aagpn2554a on my name amar nath garg. But i have not received my policy & tp card yet. Can urgently look in to the matter & provide m the same without further delay.
