I have submitted all original Reports, Bills & Doctor's advice to the Official Mr. Rajib Chakraborty on 11/3/2016. I had spent Rs. 3, 650/-(Three thousand six hundred fifty only) as per directives of National Insurance's Good health check up scheme. An amount of Rs. 1000/- (one thousand only) has been sent to my Bank A/c on 21/11/2016 . That was also made after lodging complaint. My policy no. 100600/48/15/[protected]. My earnest request to please look into the mater as I can get my claim as per directives of Good Health Check up scheme of National Insurance. Thanking you. SAJAL KUMAR GHOSH.
