Dear Sir, I the undersigned Mr. Mrudul K. Vasavada write to inform you that I am the policy holder no...310800/48/15/[protected] of National Insurance Co. Ltd. which is in the name of (1) My self Mr. Mrudul K Vasavada (2)My Wife Mrs. Himadri M. Vasavada & (3) My son Mr. Parth M. Vasavada. My wife Mrs. Himadri M. Vasavada is suffering from Right EYE Cataract & is operated in Raghudeep Eye Hospital at Ahmedabad by Dr. Abhay Vasavada on Dt. 16/10/2015.I have submitted my Claim Form on Dt .23/10/2015 along with all necessary original Documents & Receipt including Photos, I.D.Proof, Residential proof for KYC.& Blank crossed Cheque. The Operation Charges including implantation of intra ocular lens is Rs. 100500/- & Rs. 560/- is Pathology Laboratory charges .So total comes to Rs. 105560/- . The doctor has given me Rs. 10000/- discount .So my net bill amount is Rs.95560/- which I have paid & original receipt submitted to you. Str, Yesterday I have inquired about payment of my bill amount to your Baroda Office & I come to know that you have passed ONLY Rs. 24000/- but it is not credited to my Account till today . Sir, Now questions aeries (1) How much time you take to settle the CLAIM ? Even after passing the TWO months you have not made the payment..(2) I have mentioned my Present Residential Address in your prescribed form & also submitted residential proof as per your requirement as a KYC Norms you have not given me any information on my given address. Why? (3) My transactions are 100% genuine & I have to pay as per the terms & condition of the Hospital.It is also within my limit of the Policy even though you have passed ONLY Rs.24000/- WHY? Who will bare the remaining Amount ? You may inquire about my bill to the Hospital but You have to pay my bill within my limit. If I will not get my dues within 10 days I will go the consumer court against you & it is your RESPONSIBILITY.. THIS IS JUST FOR YOUR INFORMATION IN ADVANCE.
