New India Assurance — new india mediclaim policy no :[protected]

Dear sir,
I sanjeev n. Dhavan have submitted icici bank cheque no : 2838 dated 18 nov, 2017 for renewal of my mediclaim policy for insurred amount rs 700000/- (Seven lacs) with cheque amount = 21856/-. This cheque amount is handover to development officer mr. Milind kane (2d5611598) of santacruz branch, mumbai 400054. Cheque cleared on 18 nov, 2017.
The policy handover to me of insured amount rs.500000/- (Five lacs only). (Policy no :[protected]).
Inspite of continuous follow up for last one and half month and paying amount for seven lacks insured amount i am getting mediclaim policy for less amount.
It is complete failure and cheating from mr. Milind kane.
Please make detail investigation of mr. Milind kane and give me justice.
