Pacl India — how to register the refund submission form after the cutoff date

I came to know that july31-2019 is the last date for submitting the refund form on august 1st week.. Kindly help me in directing on how to submit my details and claim my refund amount. Is there any other way to submit the form or i need to contact the pacl office directly. If so where is the pacl office in erode and pls share the contact number to contact. What tare the documents a needed to exactly submit the refund form
