Pacl India — unable to submit pacl refund form

I have already submitted all the respective documents at pacl office. Now i dont have any certificate or its xerox. I just have pacl acknowledgement form and itas registration number only. Can you help me in this case. So now i have have to submit my claim as per sebi guidelines. And for this submission pacl certificate is required, so that i will be able to uplaod all required doc. I am r muthu lakshmi.Reg.No.U[protected]st receipt no.F5249748 dated on 24/06/2013 amount rs.15000/=.
I try sel[censored]pload document 18.02.18 photo, pancard, banker's cerficate in b/w&pacl certificate in colour .But no goto next stage.I pan document upoload in photo. Ack.No. Received early that day.
